Release Form Information

As of January 1, 2011, new release forms for SCA publications and web sites were required to be in use by all groups within the SCA. There are three release forms:

1. Creative Works Release for articles, artwork, poems, etc.
2. Model Release for people within a photograph
3. Photographer Release for photographs.

These forms are available as both a printable PDF and fillable PDF. Using the printable PDF the form can be completed and signed by hand and then submitted. The fillable PDF allows the form to be completed by filling in the form information on the computer and then completing the signature digitally. The form can then be saved as a PDF file to be attached and emailed for submission.

Completed forms need to be submitted to the Deputy Webwright in charge of releases. Also a copy of the form needs to be submitted to the Chronicler / Webwright of the respective publication / website that is publishing your submission. If the submission is for inclusion in the Compendium Caidis (Wiki), a copy of the form must be submitted to the Compendium Caidis Administrator.
